Due to the ongoing progression and daily changes regarding the COVID-19 outbreak in the US, the ESRD Network is requiring that all dialysis facilities that are not reporting daily at the national level complete a daily COVID-19 Facility Status Report on patients and staff members at the facility.

ALL dialysis organizations are required to complete the report DAILY unless the facility is closed.
You do not need to complete this survey if you are already reporting at the national level (i.e. Fresenius Kidney Care, DaVita, DCI, American Renal Associates, and US Renal Care)

On-going: Facility Status Reporting: COVID-19
